Who is eligible to participate?
Only Council 24 members and their families are eligible to participate in the dental plans.
When may I enroll?
If you are a current member of AFSCME Council 24, you may only enroll during open enrollment. If you are not currently a member, you may enroll in a dental plan by completing a green card to join AFSCME Council 24; a dental insurance packet will be automatically mailed to you.
What plans are available?
Council 24 offers plans through two different companies, for a total of four different plans. They are:

Where is Care Plus Prepaid available?
Care Plus is available to any member willing to go to a Dental Associates clinic (clinics are located in Appleton, Fond du Lac, Green Bay, Kenosha, and Northwest and South Central Milwaukee ). Information on this plan is automatically mailed to residents of the following counties: Brown, Calumet, Dodge, Fond du Lac, Kenosha, Milwaukee, Outagamie, Racine, Sheboygan, Waukesha, Waupaca, and Winnebago. If you live outside of this area and are interested in Care Plus Prepaid, please request CPP information from Betty Dorow at AFSCME Council 24.
What are the differences among the plans?

What is the premium cost?
Premiums vary from plan to plan. For information on 2009 plan premiums, contact Betty Dorow at bdorow@wseu-24.org or (608) 836-0024 x112.
How are premiums paid?
Premiums are paid a month in advance through payroll deduction and are taken from the "A" paycheck.
May I switch to a family plan, or add a dependent to my existing plan?
You may make changes such as these only if there has been a change in your family in the past 30 days (e.g., death, divorce, marriage, birth, adoption). Otherwise, changes such as these can only occur during the open enrollment period.
When may I cancel my plan?
You may cancel your plan at any time by completing the cancellation form and mailing it to Council 24 (8033 Excelsior Drive, Suite C, Madison, WI, 53717) or faxing it to Council 24 (608-836-0222).
I canceled my plan two weeks ago... why did I have a payroll deduction for dental insurance this month?
Because payroll centers generally process checks about two weeks in advance, you should expect to have one more payroll deduction after mailing us a cancellation form. If you have had more than one payroll deduction after mailing us your cancellation form, please notify Betty Dorow . It is often possible for payroll departments or the insurance company to reimburse erroneous deductions.
I applied for union dental insurance several weeks ago, but I still haven't received my insurance cards.
Insurance cards are administered through the individual insurance companies. Please contact your insurance company for information on the status of your dental cards.
Which dentists are covered by my plan?
A directory of Delta dentists is available on their website (www.deltadentalwi.com).
Click on "Searching for a Dentist?"
Select your plan (Delta Premier, Delta Preferred, or Delta Care) and enter the required information and your search criteria

I am leaving state service. May I make arrangements to continue my dental insurance?
You will not be eligible to continue dental insurance benefits if you resign state service, resign as a union member, or if you are promoted to a non-represented position. You may continue your plan if you are laid off or take a leave of absence. If you retire, you may continue a plan if you are covered at the time of your retirement. If I am retiring, how may I continue my dental insurance?

If I am laid off or take a leave of absence, how may I continue my policy?
